I. Information for clients and parties interested in products from the Czech-Foreigners.com website

The site Czech-Foreigners.com is owned and operated by Veridia s.r.o.

Veridia s.r.o. is a commercial company focused on development of trade operations online within the territory of Central Europe. This company owns and operates websites for arrangement of¬ sales of the products offered.

II. Material specification of business terms and conditions of Czech-Foreigners.com

These terms and conditions amend the rights and duties of contracting parties, who have entered into a contractual relationship; which means there has been a transfer of information relevant to products offered by which terms the sale of products offered via the Czech-Foreigners.com website has been initiated.

III. Business mission of Czech-Foreigners.com

The objective followed by Czech-Foreigners.com is to provide parties interested in the products on offer with coherent and up-to-date information to enable them make a qualified decision, as to whether they should purchase a certain product selected and to facilitate the sale of the appropriate products matching their needs and potential.

The information shown or communicated to parties interested in products by other means is provided directly by vendors of individual products.

IV. Product sales brokerage

The Czech-Foreigners.com website contains a list of products representing the range of products supplied by individual affiliates. The products on offer are priced by their vendors and any changes to prices may be done by particular products providers only. The provider of products shall be in no way obliged to enter into a contractual relationship with any party interested in products pursuant to the brokerage of such products. Veridia s.r.o. shall not be obliged to use the Czech-Foreigners.com for publishing of full range of products available.

Any information submitted by parties interested to the Czech-Foreigners.com are forwarded onto the relevant affiliate, who shall contact the party interested and provide them with details of particular product, actually arranging the sale of such product accordingly. Prior to conclusion of a purchase agreement with the product provider, the party interested will be made aware if all the contractual provisions and information necessary by the affiliate. Any agreements entered into by the product and provider and the party interested shall be at the sole discretion of the latter.

As far as the brokerage of product sales via Czech-Foreigners.com is concerned, the company Veridia s.r.o. cooperates with the following affiliates:

Appendix No. 1 – Insurance Terms and Conditions (Medical Insurance for Foreigners)

The said terms and conditions stipulated by Appendix No. 1 define the technical and regulatory framework for provision of information to parties interested in products and arrangement of product sales.

The parties interested in products shall be aware the software and marketing content inherent to the Czech-Foreigners.com website are subject to copyright of the company Veridia s.r.o. The party interested in products covenants to avoid such steps and/or instruments aimed at deliberate damage to the website system and the inherent information communicated to other parties interested. Any party interested shall make use of the website solely to obtain information about product(s) and arrangement of product sale agreement.

V. Personal data processing

Pursuant to provisions stipulated by the Act No. 101/2000 Coll., the Personal Data Protection Act, by completing their personal details in the draft insurance policy, the party interested has confirmed the following:

I hereby give my consent to Veridia s.r.o., business reg. ID No.: 01392867, registered office at Smotlachova 580/1, Kamyk, 142 00 Prague 4, registered in the Companies Registry administered by the Municipal Court in Prague, Section C, Insert No. 205751 (hereinafter also referred to as the "Administrator") to process my personal data provided to the Administrator in my order for product offered via Czech-Foreigners.com (especially my name, surname, personal identification number/date of birth, phone number and e-mail address) or the data obtained by the Administrator in connection with conclusion of an arranged agreement or any data provided to the Administrator pursuant to conclusion of such agreement (hereinafter referred to as the "Personal Data" only), in order to enter into an arranged agreement, fulfilment of any duties and exercise of any rights arising therefrom, in order to ensure proper administration of the arranged agreement as well as for the purpose of processing new offers or improvement of the current range of products available, for dissemination of information about new releases and new products and services offered via the Czech-Foreigners.com website. This validity of this consent shall survive the duration of agreement concluded via the Czech-Foreigners.com website. This consent may be revoked by means of a notice in writing sent to the operator of Czech-Foreigners.com. I hereby give my consent with distribution of any marketing newsletters in electronic form.

I hereby acknowledge that:

any handling of Personal Data shall be subject to provisions of the Act No. 101/2000 Coll., as subsequently amended.
The submission of my Personal Data is voluntary.
My Personal Data may be accessed by the Administrator, resp. any employees thereof as well as entities in cooperative relationship with the Administrator with respect to their agency and procurement operations in the fields of business detailed by Section IV herein (also referred to as "Other Administrators" only), while I also authorise the Other Administrators to assign my Personal Data with other personal particulars of mine; the Other Administrators shall be entitled to forward my Personal Data only to such entities necessary to participate in the arranged agreement, that is solely for the purpose intended by provisions of such arranged agreement.
My Personal Data shall be processed by the Administrator, the employees of the latter or even the Other Administrators or any employees thereof, whereas the pool of data processed by Other Administrators shall be limited to contents of the particular agreement concluded and any administrative tasks associated therewith.
My Personal Data will be processed by both automated and manual tools in terms of gathering, storage, saving on memory media, access, alterations or amendments, browsing, utilisation, distribution, publishing, archiving, exchange, sorting or combining, barring and disposal.
I have the right to enquire with the Administrator to obtain information relevant to processing of my Personal Data (esp. the purpose of processing, personal details subject to processing, the type of automated tools for Personal Data processing).
Should I find out or have any reasons to suspect the Administrator from processing of my Personal Data by any means violating my privacy protection or contrary to the legal regulations in place, especially if the Personal Data processed is inaccurate with respect to the processing purpose, I shall have the right to ask the Administrator for explanation or disposal of my Personal Data accordingly.
Should the Administrator fail to grant my request for explanation or remedy, I shall have the right to approach the Office for Personal Data Protection (I am entitled to approach this Office directly as well).
I shall have the right to request the Administrator to remove my Personal Data from the database, whether in full or to a partial extent only, except for the details vital for administration of the arranged agreements or other products and services already arranged via the Administrator.

VII. Final provisions

Veridia s.r.o. reserves the right to change any of the business terms and conditions at its sole discretion. No invalid or ineffective provisions shall affect the validity of the remaining business terms and conditions as a whole, the remaining parts of the business terms and conditions respectively. These business terms and conditions have been executed in writing only and the up-to-date version shall be available at the Czech-Foreigners.com website.

Appendix No. 1– Insurance Terms and Conditions (Medical Insurance for Foreigners)

I. Information for clients and parties interested in insurance

The Intermediary conducts their business operations as an insurance intermediary in terms of the Act No. 38/2004 Coll., on Insurance Intermediaries and on Independent Loss Adjusters, comprising the main line of business pursued by the Intermediary.

Registration of Intermediary
The Intermediary is registered in as an insurance broker and insurance agent under the above-mentioned registration numbers held in the registry maintained by the Czech National Bank. The registration can be verified at: http://ispoz.cnb.cz .

Declaration of Independence

The Intermediary hereby declares it has no direct or indirect influence exceeding 10 % of total voting rights or equity in any insurance company involved in arrangement of insurance policies for the client.
The Intermediary hereby declares that no insurance company involved in arrangement of insurance policies for the client owns any direct or indirect share exceeding 10 % of voting rights or equity of the Intermediary.

Affiliated Insurance Companies

The Intermediary shall be entitled to submit offers, negotiate and enter into insurance policies with the insurance companies listed below:

Method and handling of complaints

Any inconvenience attributable to the procedure followed by the broker and experienced by the client or another entity affected may be reported in writing at the address below: MONDE FINANCE s.r.o., Narodni 138/10, 110 00 Prague 1 or the e-mail address: pojisteni@mon¬de.cz, or even the Department of Control and Supervision of Insurance Providers at the Czech National Bank. Complaints can be submitted in writing at the following address: Czech National Bank, Financial Market Sueprvision Department, Na Prikope 28, 115 03 Prague 1 or via e-mail at: podatelna@cnb.cz. Any claims towards the Intermediary or underwriter(s) submitted by the beneficiary may be also enforced by legal means in compliance with the Act No. 99/1963 Coll., the Code of Civil Procedure and other legal regulations.

Intermediation of insurance policies

The Intermediary shall be entitled to negotiate insurance policies in accordance with the Act No. 38/2004 Coll. The information provided by the client when submitting details of their requirements and needs via the electronic form or disclosing such data by electronic means with their application for insurance will be used to process a reference for conclusion of insurance policy. The requirements and needs presented by the party interested in insurance are recorded in the electronic form on the reference for conclusion of insurance policy.

Getting your insurance policy

The data you submitted will be verified by our insurance expert, who will validate the insurance policy, otherwise the policy remains void until validated. Once the policy has been processed, you will receive a notification of its dispatch by e-mail.

An insurance policy shall be considered executed upon settlement of the premium at the amount and within the period stated in the policy draft. Further information for parties interested any policy holders regarding the insurance can be obtained through the website of the Intermediary or underwriter, or even via the e-mail address of the Intermediary at: pojisteni@monde.cz. Any party interested in insurance shall have the right to request further information relevant to insurance from the underwriter.

Insurance policy details

Each insurance policy shall be governed by the law of Czech Republic. Any wording of policies and communication between contracting parties shall be in Czech language. Once concluded, each insurance policy shall be filed by the underwriter for at least the period applicable for provision of supplies and fulfilment of obligations arising therefrom. Each policy holder shall be entitled to request a copy of their policy from the underwriter throughout the policy duration.

Insurance coverage and premiums

The insurance shall be provided in compliance with the insurance terms and conditions and the applicable contractual provisions. These documents refer to the detailed product characteristics too. The scope and description of insurance, the duration and the method for payment of premiums are stipulated in the draft insurance policy, the terms and conditions of insurance as well as the relevant contractual provisions. For details on methods for provision of services, as well as the termination of insurance policy, please refer to the terms and condition of insurance of the legal amendment of the insurance policy respectively.

II. Personal data processing

Pursuant to provisions stipulated by the Act No. 101/2000 Coll., the Personal Data Protection Act, by completing their personal details in the draft insurance policy, the party interested has confirmed the following:

ID No.: 26142554, registered office at Smotlachova Narodni 138/10, New Town, 110 00 Prague 1, registered in the Companies Registry administered by the Municipal Court in Prague, Section C, Insert No. 73983 (hereinafter also referred to as the "Administrator") to process my personal data provided to the Administrator in my order for insurance (especially my name, surname, personal identification number/date of birth, phone number and e-mail address) or the data obtained by the Administrator in connection with conclusion or performance of the policy or any data provided to the Administrator pursuant to conclusion of such policy (hereinafter referred to as the "Personal Data" only), in order to enter into an insurance policy, fulfilment of obligations and exercise of duties arising therefrom, as well as for the purpose of administration of the insurance established pursuant to such policy (if actually concluded). This consent has been granted for the duration of insurance policy and further period of three years beyond expiration thereof (if actually concluded) or for the period of three years pursuant to provision of this consent (if no policy has been concluded).

My Personal Data shall be processed by the Administrator, their employees or even Other Administrators or their employees respectively.
My Personal Data will be processed by both automated and manual tools in terms of gathering, storage, saving on memory media, access, alterations or amendments, browsing, utilisation, distribution, publishing, archiving, exchange, sorting or combining, barring and disposal.
I have the right to enquire with the Administrator to obtain information relevant to processing of my Personal Data (esp. the purpose of processing, personal details subject to processing, the type of automated tools for Personal Data processing).
